Which Plan am I eligible for?

The Health Fund offers five plans to participants:
36-phlex-workers

36 Phlex Plan

The 36Phlex Plan is a health and welfare benefits plan that gives you the freedom to choose from a menu of benefits options. The various coverage options ensure that you can create a benefits package that meets your specific needs.

BOLR Full-Time Plan

The BOLR Full-Time Plan offers you and your eligible dependents medical, prescription drug, dental, and vision benefits. You also have access to Life and Accidental Death and Dismemberment (AD&D) insurance, in addition to disability income protection. Please note that the Death Benefit is only available to the member or employee and not to dependents.

BOLR BMCA Full-Time Plan

The BOLR BMCA Full-Time Plan offers you and your eligible dependent child or children medical, prescription drug, dental, and vision benefits. You also have access to Life and Accidental Death and Dismemberment (AD&D) insurance and disability benefits. Please note that the Death Benefit is only available to the member or employee and not to dependents.

The BOLR Part-Time and Weekend Worker Plan offers you and your eligible dependents dental and vision benefits. You also have access to Life and Accidental Death and Dismemberment (AD&D) insurance and disability benefits. Please note that the Death Benefit is only available to the member or employee
and not to dependents.

BOLR BMCA Part-Time Plan

The BOLR BMCA Part-Time Plan offers you and your eligible dependents dental and vision benefits. You also have access to Life and Accidental Death and Dismemberment (AD&D) insurance and disability benefits. Please note that the Death Benefit is only available to the member or employee
and not to dependents.

Eligibility for each Plan varies based on your collective bargaining agreement and your employment status. Check your collective bargaining agreement to see which Plan(s) are available to you. If you have questions, contact the Fund Office.

Do you live out of the area?
If you live in Pennsylvania but outside of the Philadelphia geographic area or out of state, your benefit plan will be a little different. Please call the Fund Office for information about the options available to you.
